Lee Camp, No. 1, Confederate Veterans At Richmond, Virginia I, Daniel Hurst, of the County of Pulaski State of Virginia set forth that I, the said Daniel Hurst was regularly enlisted and served, during the late war between the States, in the Confederate States army or navy as follows: When Enlisted, with Rank April 1861, Sergeant Where Enlisted, Town and State Cumberland Gap, Claiborne Co., Tenn. Company and Regiment Mustered in Branch of Service Co. "A" 63 Tenn Regt. Date and Place of Discharge with Rank July 18, 1865 Point Lookout, Md. Cause of Discharge Discharged from Prison. If wounded, give date and name of engagement. Fishing Creek, Ky., Dec. 18th 1861 Chicamauga, Tenn Sept. 20, 1863. Drewery’s Bluff, Apr. 16, 1864. State if you volunteered; were you conscripted, or were you a substitute? Volunteered. I served honorably during the war in said army (or navy), and was true and loyal to the government of the Confederate States to the end of the war, and I have borne a good character ever since; I am 72 years of age; I am widower; I am of fair complexion, 5 ft. 7" in height, and am by occupation a Farmer; my nearest relative is Caloway Hurst whose postoffice is Grapevine, Texas; that because of wounds, infirmities of age, I the said Daniel Hurst am unable to provide for and support myself, and therefore I make this my application for admission into the "Soldiers’ Home": and I swear or affirm that I am a person of sobriety and good habits, and, if admitted as an inmate to said Home, I hereby agree to abide by and obey all the rules and regulations made by the proper authorities of the Home for its government and that I will perform all duties required of me, and obey all lawful orders of the officers of the said Home; and I sign this application with the full knowledge that any violation of the rules will subject me to punishment or dismissal, at the discretion of the Board of Visitors.
